Combine with camera recognizable tags?
Seems like it would be a perfect mashup with the photo tags that take you right to a site. Click the picture on the box, fill out the questions, done.

Do you ever check to determine if respondent knows anything about the subject of the survey. The opinion of an idiot doesn't impres me much.

Pollmate Is Better
Pollmate is another great survey tool. We chose pollmate as it just seemed more professional and for the results we where looking for.. They also donate money from each survey answered to charity which is nice.

Mobile / On the spot is indeed the future of Survey
Great Videos showing how mobile survey can indeed deliver more useful insight than traditional web tools.

A strong solution with native mobile application is Datafield www.data-field.com. There's indeed the ability to collect also picture, digital signature, GPS position ...
