Rethinking Healthcare

An anonymous doctor reports from the front

By Dana Blankenhorn | Jul 29, 2009 |

Forget Scrubs, or even the late, lamented ER.

If you want the real skinny on working in a modern American hospital, head over to Anonymous Doc.

The author identifies himself (or herself) only as a “first-year internal medicine resident at a big-city hospital. Or not.”

So this is either a real intern, or a great writer posing as one, because everything in this blog scans true. (And forget the picture for clues. It’s Neil Patrick Harris as Doogie Howser, MD.)

Here’s a post that reads like a Scrubs episode:

I’m tired. I’m seeing myself getting used to people dying, which is uncomfortable and scary and not the way I want to feel. But how can I avoid it? Death, in a hospital setting, is unfortunately routine. I don’t want it to be, but already it’s feeling like in a lot of cases, it’s not even such a bad thing.

Here’s a more intense piece that drew dozens of comments:

The patient has no signs of dementia, seems mentally competent.

The attending agreed to do what the son was asking. So now we’re all calling this thing an “infection” and telling her the chemotherapy drugs are “antibiotics”.

I find this appalling.

The comments can be as good as the posts. “Unequivocally ethically wrong” writes a British doctor about the decision not to tell the patient about her cancer. “My dad did this with my mom,” someone quickly replied.

It’s one of those great products of the Internet, something that couldn’t possibly exist without it. For me it’s a guilty pleasure, like a good book or a TV show like “Mad Men” I know I shouldn’t like but can’t keep from watching.

Check it out and see if you don’t agree. And if this intern is reading this, you don’t have to look at death ever again if you don’t want to. After your residency is over, head to Hollywood. Your portfolio is done.

RE: An anonymous doctor reports from the front

I had to be tough to work around death and crime. I can relate, but I'm no doctor. I would ask myself if I would see anything good each day, but my way of coping was to find someone that I COULD help. Rethink healthcare, yes... even if you don't realize it at the time, it can catch up with you later. I have many crazy and scary stories from working as a cop, and I didn't even like cops. Too many of them seemed to want to be gods, like some doctors do. I wanted to help others. Dead bodies, people inside burning houses, babies being abused, corruption in the courts were part of the real job. Vivid memories of what I thought would not bother me, eventually caught up. I worked the entire county where I live. Memories of death or abuse come to me just by driving around. Rethink healthcare, and rethink law enforcement. Firefighters, and related fields all have my empathy. Think again. memories can come back like a soldier who has PTSD. It not only hurts you, it can also hurt your family. Then again, someone has to do it. Yes, I should head to Hollywood. Not many would believe my stores, maybe the Syfy channel could use some help.

    Thanks for writing

Many people other than soldiers have PTSD. I think what this doctor
indicates is that anyone who has gone through residency may fall victim to
it. As many any cop or firefighter.

    comparison with my blog

I'm a practicing surgeon in a third world country. And from the short glance at the latest entries on that blog, I see some familiar scenes.

    Here's my blog

    http://magos-biologis.blogspot.com/

As you can see if you go there, its mostly computer/tech stuff. I dont watch ER or scrubs and I dont blog much about my practice because I want blogging to be a time when I can forget about my day job and talk about my hobby.

I used to watch Chicago Hope because it was not serious, more of a comedy so I could lagh at my day job (I was still in training then).
